MyGuestSurvey.com was developed by Code3w, Inc. for use by Participating Businesses and their Guests. Guests and Participating Businesses are given an Invitation Code with which they may access the website, subject to the terms of these Privacy Policies.
Privacy policies may be modified at any time. Any changes will be posted within this page, and identified as changes to it.
After answering survey items, Guests of Participating Businesses may choose to receive a coupon from that business.
Participating Businesses may modify coupon offers or discontinue use of MyGuestSurvey.com at any time. They may choose to honor, or choose not to honor, any coupons that have been sent prior to discontinuing the website use.
Code3w, Inc. respects the privacy of every individual who visits MyGuestSurvey.com.
No credit card numbers or social security numbers are collected on MyGuestSurvey.com.
The email addresses input by Guests and Participating Businesses are stored in a database at a 24/7 secured facility. Guest survey responses are securely and anonymously stored as well.
Code3w, Inc. reserves the right to disclose, as governed by law, any information required in compliance with a judicial proceeding, court order or legal process.
To participate in a survey, the Guest must first input an Invitation Code, provided by a Participating Business.
Survey item reports consist of totals and averages. Individual survey responses are not provided to the Participating Business. Those individual responses are confidential.
If a Guest completes a survey but leaves the website without selecting a coupon, that Guest will receive no coupons or further communication. Survey results will be included in the totals and averages reported to the Participating Business.
Each of the Participating Businesses at MyGuestSurvey.com offers single coupons or the Coupon Club option, where coupons are sent periodically.
Participating Businesses distribute the Guest Invitation Code and are the source and authority for coupon content. MyGuestSurvey.com communicates but does not itself offer coupons. It will not reimburse any value for a coupon. Only the Participating Business may honor a coupon, and the coupon will be subject to the terms printed on it.
Coupon terms may change without notice. A Participating Business may not honor a coupon due to product availability, business policy change or any other reason.
If a Guest chooses to receive one coupon, that coupon will be sent one time. The Guest will receive no further communications from MyGuestSurvey.com unless a business participant gives another Invitation Code to that Guest.
A Guest who returns to MyGuestSurvey.com for subsequent visits may not reuse an Invitation Code to receive a coupon at the same email address. Only one coupon will be sent per Invitation Code and email address, unless the Guest chooses to join the Coupon Club.
If a Guest chooses to join the Coupon Club, that Guest may receive further coupons from the Participating Business.
After completing a survey, Guests are asked to provide their email address. The Guest must opt-in to this request in order to receive a coupon.
Guest email addresses are used only to distribute coupons. All coupons are distributed via email.
Each coupon email includes an opt-out link to a MyGuestSurvey.com page. On that page, the Guest can confirm the request to opt-out. Once this request is confirmed, the Guest email address is removed from the MyGuestSurvey.com email list for that Partnering Business.
A clear .GIF file may be used to identify if a Guest email has been opened. The Guest can discontinue this by choosing to opt-out.
Guests who input Invitation Codes from multiple Participating Businesses and then take surveys for those businesses may receive email from each of those businesses. Each email includes a separate opt-out link. The Guest may choose to opt-out from one business, but continue to receive coupons for another business.
Participating Business email addresses are used for routine correspondence including but not limited to billing status, website maintenance notification, and upgrade announcements.
While using the website, Guests or Participating Businesses may be asked to refer a friend or other business to MyGuestSurvey.com. If a Guest or Participating Business chooses to act as the referrer, the referrer name and email address must be input along with the referral email address.
Code3w, Inc. will send a one-time email to the referred email address. The email will include the referrer email address as source of the referral, and will invite a visit to the website. Like all email from MyGuestSurvey.com, an unsubscribe link will be provided. No further email will be sent unless requested.
Code3w, Inc. assumes no responsibility for direct or indirect result of any action or inaction taken based on the surveys, coupons and other information on this website.
No warranty for damage or loss is offered to any Participating Business or Guest.
Code3w, Inc. strives to keep this website current, accurate and complete but does not guarantee timeliness, accuracy or completeness of the content.
The Participating Business is responsible for distributing Guest Invitation Codes. In compliance with the Children’s Online Privacy Protection Act, Code3w requires the Participating Business to distribute Invitation Codes only to persons age 13 or older.
No changes have been posted to this Privacy and Use Policy since its original publication on February 17, 2009.
